As Mckee say: If the protagonist want he object of desire he will need to do things. Hard things. Maybe lie to people (Wedding Crashers), put his own life in jeopardy (Die Hard), choose to let people die (two times in Die Hard), steal a car (Speed/The Next three days), or kill the person will love (Million Dollar Baby).

Most of the times character to achieve what they want will do illegal actions (The Next Three days/Crash/Million Dollar Baby), danger things (put his own life in jeopardy like armed guy without holding a gun), imoral things (lie).

The dilemma works because all this actions is motivated by the object of desire.
